The thyroglobulin antibody test measures antibodies against thyroglobulin, a protein your thyroid uses to make hormones. Like TPO antibodies, elevated thyroglobulin antibodies indicate autoimmune thyroid disease. Normal thyroglobulin antibody levels are typically less than 1 IU/mL. Interestingly, some people can have elevated antibodies but normal hormone levels and no symptoms—a condition doctors call subclinical autoimmune thyroiditis. In these cases, doctors may monitor thyroid function over time without starting treatment.

The TSI or TRAb test measures thyroid receptor antibodies, specifically those that bind to TSH receptors on your thyroid cells. This test is ordered when hyperthyroidism is suspected, particularly Graves' disease, an autoimmune condition where antibodies stimulate your thyroid to produce excess hormones. Normal TSI levels are less than 1.75 IU/L. Elevated levels confirm Graves' disease and may indicate disease severity. This test is particularly valuable during pregnancy because TSI antibodies can cross the placenta and affect the fetus.
