The prostate is controlled by hormones, particularly testosterone. This hormone connection means that prostate health is linked to overall hormonal balance and aging in men. The gland performs its reproductive function for most of a man's life, but problems can develop at any age, though they become more common after age 50.

Several conditions can affect the prostate. Benign Prostatic Hyperplasia (BPH) is enlargement of the prostate that happens with aging. Prostatitis is inflammation or infection of the prostate. Prostate cancer occurs when malignant cells develop in the prostate tissue. Each condition has different causes, symptoms, and treatment approaches.

Prostatitis is inflammation of the prostate, which can be caused by bacterial infection or other factors that are not yet fully understood. There are several types: acute bacterial prostatitis (sudden, severe infection), chronic bacterial prostatitis (recurring infection), chronic prostatitis/chronic pelvic pain syndrome (pain without proven infection), and asymptomatic inflammatory prostatitis (inflammation without symptoms). Symptoms can include pain during urination, difficulty urinating, pain in the lower back or between the scrotum and rectum, painful ejaculation, and sometimes fever and chills in acute cases.

Prostate cancer develops when malignant cells grow in the prostate tissue. Many men with early-stage prostate cancer have no symptoms at all. When symptoms do appear, they may include difficulty urinating, weak urine stream, frequent urination (especially at night), blood in urine or semen, pain during ejaculation, and discomfort in the lower back, hips, or upper thighs. It is important to note that these symptoms can also occur with non-cancerous conditions, so symptoms alone do not indicate cancer.

Family history significantly increases prostate cancer risk. A man whose father, son, or brother had prostate cancer is twice as likely to develop the disease. This risk is even higher if multiple family members were diagnosed, if relatives were diagnosed at a young age (before 65), or if the family has a history of hereditary prostate cancer syndromes. Men with a family history should consider discussing screening with their doctors earlier than standard recommendations—sometimes as early as age 40.

Race and ethnicity also play a role in prostate cancer risk. African American men have the highest incidence and mortality rates from prostate cancer in the United States. They are about 1.5 times more likely to be diagnosed with prostate cancer and are more than twice as likely to die from it compared to white men. Men of Scandinavian descent also show higher rates. The reasons for these disparities are complex and involve both genetic factors and differences in access to healthcare screening.

Other risk factors include obesity, physical inactivity, and diet high in red meat or dairy products. Some research suggests that certain genetic mutations may increase risk, though genetic testing for prostate cancer risk is still evolving and not yet standard practice. Hormone levels, particularly testosterone and insulin-like growth factor, may also play a role, though this relationship is still being studied.

The exam is brief, usually lasting only a few seconds. While some men feel embarrassed about this exam, it is a standard medical procedure. A DRE can detect some prostate abnormalities that might not show up on a PSA test.

Transrectal ultrasound (TRUS) uses sound waves to create images of the prostate. A probe is inserted into the rectum to allow the ultrasound to get close to the prostate tissue and create detailed pictures. This test may be performed if a PSA test is elevated or if a doctor suspects a problem based on symptoms or physical examination. TRUS takes about 20 minutes and is generally not painful, though some men find it uncomfortable.
