During childhood and early adulthood, new bone is created faster than old bone is removed, which increases bone mass. Most people reach their peak bone mass around age 30. After that, bone remodeling continues, but more bone is lost than gained.

The development of osteoporosis happens gradually and without symptoms in most cases. You cannot feel your bones becoming weaker. This is why osteoporosis is often called a "silent disease." Many people do not realize they have the condition until they break a bone after a minor fall or injury. Common fracture sites include the hip, spine, and wrist.

If you cannot meet your calcium needs through food, calcium supplements are available, though getting nutrients from food is generally preferred when possible.

Physical activity builds and maintains bone strength. Weight-bearing exercises—where your bones and muscles work against gravity—are particularly beneficial. These include walking, jogging, dancing, hiking, and stair climbing. Muscle-strengthening activities like resistance training, weight lifting, and bodyweight exercises also help. Research shows that people who engage in regular physical activity have higher bone density than sedentary individuals. The U.S. Department of Health and Human Services recommends at least 150 minutes of moderate-intensity aerobic activity per week for adults, combined with muscle-strengthening activities on two or more days per week.

Several lifestyle habits negatively affect bone health. Smoking decreases bone density and increases fracture risk. Alcohol consumption, particularly heavy drinking, can interfere with calcium absorption and damage bone tissue. Excessive caffeine intake may slightly reduce calcium absorption, though moderate caffeine consumption (a few cups of coffee daily) is generally not considered harmful to bone health. Limiting sodium intake is also important, as high sodium consumption can increase calcium loss through urine.

Body weight influences bone density. Being underweight increases osteoporosis risk because there is less weight bearing on bones, and underweight individuals often have lower overall nutrition. However, obesity does not necessarily mean stronger bones—excess weight can sometimes mask low bone density, and obese individuals may still have poor bone quality and fracture risk.
