The process starts when pressure from body weight cuts off blood supply to an area of skin. This commonly happens over bony parts of the body like the tailbone, heels, hips, elbows, and shoulder blades. When skin doesn't receive oxygen and nutrients from blood flow, the cells begin to die. The damage can progress quickly—sometimes in just a few hours if pressure remains constant. Unlike injuries you can see immediately, bed sores often develop beneath the surface first, making early detection challenging.

According to the National Pressure Injury Advisory Panel, bed sores affect between 1 and 3 million Americans annually. The risk increases significantly for people who are bedridden, use wheelchairs, or have limited mobility. People over 70, those with spinal cord injuries, and individuals with diabetes or poor circulation face higher risk. The condition is particularly common in hospital and long-term care settings, where one study found that approximately 8% of hospitalized patients develop pressure injuries during their stay.

Several factors beyond immobility contribute to bed sore development. Moisture from sweat, urine, or other bodily fluids breaks down skin and makes it more vulnerable to damage. Poor nutrition, especially insufficient protein and vitamin C, weakens the skin's ability to repair itself. Friction from rubbing against sheets or being dragged across surfaces damages the outer skin layer. Additionally, shearing forces—when one layer of skin slides against another—can separate tissues and create conditions where bed sores develop.

The affected area may feel warm, hard, or soft to the touch compared to surrounding skin. For people with darker skin tones, the discolored area may appear purple, blue, or darker than surrounding skin rather than red. This stage can be easy to miss because many people and even some healthcare workers may dismiss it as simple redness that will go away on its own.

Stage 2 injuries involve partial thickness skin loss. The top layer of skin (epidermis) and part of the underlying layer (dermis) have been damaged. These wounds may look like blisters, open sores, or scraped areas. They may weep clear fluid or appear shiny and moist. Pain is common at this stage. The wound may start to smell if infection develops. At this point, the damage has progressed beyond the surface and requires active intervention to prevent further deterioration.

Stage 3 and 4 injuries involve deeper tissue damage that may include fat, muscle, and bone. At these advanced stages, the wound may have a sunken appearance and could contain discolored tissue or a dark, hard scab-like substance called eschar. These wounds often develop a strong odor and may drain fluid. The surrounding skin may appear bruised or discolored. These advanced stages carry serious risk for infection and sepsis and require professional medical care.

Beyond the four defined stages, there are two additional categories. Unstageable pressure injuries occur when the wound base is covered by eschar or slough (dead tissue), making it impossible to see how deep the damage goes. Deep tissue injuries appear as a localized area of purple or maroon discoloration of intact skin or a blood-filled blister. These can expand rapidly and may indicate serious damage beneath the surface.

Practical takeaway: Check daily for unexplained redness that doesn't fade when pressed, especially over bony areas. Early intervention at the first sign of redness can prevent progression to more serious stages requiring intensive treatment.

Prevention Strategies for Mobility-Limited Individuals

The most effective approach to bed sores is prevention. People with limited mobility can remain free of pressure injuries by following consistent prevention practices. These strategies work by addressing the main causes: relieving pressure, managing moisture, reducing friction, and supporting overall health.

Repositioning is the cornerstone of pressure injury prevention. People who are bedridden should change position at least every 2 hours. Those in wheelchairs need to shift weight or be repositioned every hour. A simple repositioning schedule posted at the bedside or on a wheelchair helps ensure consistency. When repositioning, lift the person rather than dragging, which reduces friction and shearing. Use pillows or foam wedges to keep bony areas from touching each other. For example, place a pillow between the knees to prevent the bones of the knees and ankles from pressing together. Keep heels off the bed surface entirely by using a pillow under the calf—never place pillows under the knees as this increases pressure on the heel.

Managing moisture prevents skin breakdown. Incontinence products should be checked frequently and changed immediately when wet or soiled. Bathe with warm (not hot) water and mild soap, then pat dry thoroughly, paying special attention to skin folds. Use moisture barriers or skin protectants on areas prone to wetness. Minimize sweating by keeping the room at a comfortable temperature and using breathable, moisture-wicking bedding. If a person sweats heavily during sleep, change sheets and nightclothes promptly.

Nutritional support strengthens skin and tissue. Adequate protein helps rebuild damaged tissue—the recommended intake is 1 to 1.5 grams per kilogram of body weight daily. Vitamin C supports collagen formation and healing. Zinc assists in tissue repair. If someone cannot meet nutritional needs through food alone, supplements or specialized nutrition drinks may be recommended by a healthcare provider. Maintaining adequate hydration by drinking water throughout the day also supports skin health.

Use pressure-relieving surfaces like high-density foam mattresses, gel overlays, or air mattresses that distribute weight more evenly than standard mattresses. Static surfaces (that don't move) work for prevention in mobile individuals, while dynamic surfaces with active pressure relief are better for those at very high risk or with existing wounds. However, these surfaces supplement but do not replace repositioning—they are not a substitute for changing position regularly.

Protect skin from friction and shearing by using draw sheets (special turning sheets) to move people rather than dragging them across regular sheets. Keep bed linens clean, dry, and wrinkle-free. Use protective padding on bedrails or chair arms that the person might rub against. Choose clothing and bedding without rough seams or irritating textures.

Daily inspection should be thorough and systematic. Check all areas at risk, especially those over bony prominences. Use a mirror to check the lower back and buttocks. Look for redness, discoloration, warmth, swelling, or any break in the skin. Press gently on red areas with your finger—if the redness doesn't fade within a few seconds, this indicates a Stage 1 pressure injury. Document what you see by noting the location, appearance, and size of any concerning areas. Taking photos with a date and time stamp creates a useful record for tracking changes over time.
