Women are less likely to develop kidney stones than men overall, but the number of women experiencing kidney stones has been rising over the past 20 years. In fact, kidney stone disease now affects approximately 1 in 11 Americans, with rates increasing for both genders.

Women face unique risk factors for kidney stone formation. Dehydration is one of the most common causes—when you don't drink enough fluids, your urine becomes more concentrated, making it easier for stones to form. Certain medications can increase kidney stone risk, including some diuretics and vitamin supplements. Women with a history of urinary tract infections may be at higher risk, as infections can alter the chemical balance in urine. Additionally, women with inflammatory bowel diseases like Crohn's disease or ulcerative colitis have increased rates of kidney stone formation because these conditions affect how the body absorbs water and minerals.

Diet plays a significant role in kidney stone development. A diet high in sodium, animal proteins, and foods containing oxalate (such as spinach, nuts, and chocolate) can contribute to stone formation. Women who consume excessive amounts of vitamin C through supplements may also increase their risk, as the body converts excess vitamin C into oxalate. Metabolic disorders and genetic factors can also predispose some women to kidney stones, meaning the condition may run in families.

Understanding your personal risk factors is the first step toward prevention. If you have a family history of kidney stones, experienced one before, or have certain medical conditions, you may want to discuss your individual risk with a healthcare provider. Keeping track of your hydration habits, medication use, and dietary patterns can help you identify which factors might apply to your situation.

Recognizing Symptoms and When to Seek Medical Attention

Kidney stone symptoms can vary depending on the size and location of the stone. Small stones may pass through your urinary system without causing noticeable symptoms, sometimes called "silent stones." However, larger stones often cause sudden, intense pain called renal colic. This pain typically begins in the side or back, below the rib cage, and can move down toward the lower abdomen and groin as the stone moves through the urinary tract. Women sometimes describe kidney stone pain as similar to labor contractions—it can come and go in waves.

Other common symptoms accompanying kidney stones include blood in the urine (which may appear pink, red, or brown), nausea and vomiting, burning sensation during urination, persistent urge to urinate, and cloudy or foul-smelling urine. Some women experience fever and chills, which may indicate an infection. The severity of pain doesn't necessarily reflect the size of the stone; even small stones can cause significant discomfort if they irritate the urinary tract lining.

You should seek medical attention if you experience severe pain in your back, side, lower abdomen, or groin; blood in your urine; fever above 101 degrees Fahrenheit; inability to urinate; persistent nausea and vomiting; or pain accompanied by a urinary tract infection. If you have a single kidney or are pregnant and suspect a kidney stone, medical evaluation is particularly important. Don't wait to see if the pain resolves on its own if you have fever or signs of infection, as kidney stone complications can include urinary tract infections, kidney damage, and urine backup.

When you visit a healthcare provider, describe your symptoms in detail, including when they started, how severe the pain is on a scale of 1-10, what makes the pain better or worse, and any other symptoms you're experiencing. Mentioning any previous kidney stones, family history, recent travel (which might have affected hydration), or dietary changes can help with diagnosis. Taking note of whether you've noticed changes in your urine appearance or frequency is also helpful information to share.

Diagnosis and Testing for Kidney Stones

Several tests can confirm whether you have a kidney stone. The CT scan (computed tomography) is considered the gold standard for kidney stone diagnosis because it can detect nearly all types of stones and can also identify complications. This imaging test takes detailed cross-sectional images of your abdomen and pelvis, allowing doctors to see the exact location, size, and number of stones. CT scans are fast—typically taking just a few minutes—and don't require dye injection, though some versions do use dye.

An ultrasound may also be used to detect kidney stones, particularly in pregnant women when radiation exposure should be minimized. Ultrasound uses sound waves to create images and can show larger stones effectively, though it may miss very small ones. X-rays can detect some calcium-based stones (which make up about 80 percent of all kidney stones) but may not show uric acid or other less dense stones. Your healthcare provider will choose the imaging method based on your specific situation, medical history, and any pregnancy considerations.

A urinalysis (urine test) can reveal blood, white blood cells, bacteria, or crystals in your urine that suggest kidney stones or infection. Blood tests may show elevated kidney function markers or electrolyte imbalances. If you pass a stone or have one removed during treatment, analysis of the stone composition helps determine its type (calcium oxalate, calcium phosphate, uric acid, or other types). This information is valuable for developing a prevention strategy tailored to why your stones formed.

After diagnosis, your healthcare provider may ask about your medical history, current medications, dietary habits, and fluid intake to understand what may have caused the stone. They might also recommend a 24-hour urine collection test, where you collect all urine over 24 hours to measure the levels of stone-forming substances in your urine. This test helps identify metabolic abnormalities and guides prevention recommendations. Understanding your stone type and the underlying cause is essential for developing an effective prevention plan to reduce your risk of future stones.

Treatment Options for Kidney Stones

Treatment for kidney stones depends on the size, type, location, and whether the stone is causing complications. Many small stones (less than 5 millimeters) pass on their own, typically within a few weeks. During this time, pain management and increased fluid intake are the main treatment approaches. Over-the-counter pain relievers like ibuprofen or naproxen can help manage discomfort, though some women require prescription-strength pain medication if the pain is severe. Staying well-hydrated helps flush your urinary system and may help facilitate stone passage.

For larger stones or those causing severe symptoms or complications, medical intervention may be necessary. Extracorporeal shock wave lithotripsy (ESWL) is a non-invasive procedure that uses shock waves to break kidney stones into smaller fragments that can pass through the urinary system more easily. The procedure typically takes 45 to 60 minutes, and you can usually go home the same day. ESWL works best for stones less than 2 centimeters in diameter and may require multiple sessions.

Ureteroscopy is another common treatment, particularly for stones in the ureter (the tube that carries urine from the kidney to the bladder). During this procedure, a thin tube with a camera is inserted through the urethra and bladder into the ureter. The surgeon can visualize the stone and either remove it or break it into smaller pieces using a laser. This is usually done under general or regional anesthesia and may be outpatient. Percutaneous nephrolithotomy is used for larger stones (usually over 2 centimeters) and involves a small incision in the back through which instruments are passed directly to the kidney to remove the stone.

If a stone is infected or blocking urine flow and causing kidney damage, treatment is urgent. In these cases, a stent (small tube) may be temporarily placed in the ureter to allow urine to drain past the stone until definitive treatment can be performed. Your healthcare provider will discuss which treatment option is most suitable based on your stone characteristics, overall health, and preferences. Recovery time varies from same-day discharge with lithotripsy to several days for more invasive procedures.

Prevention Strategies Specific to Women
