Lupus affects approximately 1.5 million Americans, though some estimates suggest the number could be higher due to underdiagnosis. About 90 percent of people diagnosed with lupus are women of childbearing age, typically between 15 and 44 years old. However, lupus can develop in men, children, and older adults as well. The disease is more common in African American, Hispanic, Asian, and Native American populations compared to people of European descent.

Testing for lupus is important because early detection can lead to earlier treatment, which may help prevent serious organ damage. Lupus can affect the kidneys, causing kidney disease, and can increase the risk of heart disease and stroke. The unpredictable nature of lupus makes diagnosis challenging. Symptoms vary widely from person to person and can mimic other conditions like rheumatoid arthritis, Lyme disease, or fibromyalgia. Some people experience mild symptoms that come and go, while others develop serious complications. Fatigue is one of the most common complaints among people with lupus, affecting up to 80 percent of patients. This fatigue is often severe and not relieved by rest. Many people describe it as overwhelming exhaustion that interferes with daily activities. Joint and muscle pain is another frequent symptom, often affecting the hands, feet, knees, and wrists. Unlike rheumatoid arthritis, lupus typically does not cause permanent joint damage, though the pain can be significant.

Lupus can cause fever without an obvious infection, usually between 98.5 and 101.5 degrees Fahrenheit. Some people experience Raynaud's phenomenon, where fingers and toes turn white, blue, or red in response to cold or stress due to poor circulation. Anti-DNA and anti-Smith (anti-Sm) antibody tests are more specific to lupus. The anti-Smith antibody test is particularly useful because it is found in about 20 to 30 percent of lupus patients and is rarely found in other conditions. Anti-double-stranded DNA antibodies are found in 60 to 70 percent of lupus patients and tend to correlate with kidney involvement and disease activity. These tests provide stronger evidence for lupus diagnosis than the ANA test alone.

The complement system is part of the immune response that helps clear pathogens and dead cells. In lupus, complement levels often decrease because the body uses up these proteins in the inflammatory process. Doctors measure complement 3 and complement 4 levels through blood tests. Low complement levels support a lupus diagnosis and may indicate more active disease.

Additional blood work includes a complete blood count (CBC) to measure red blood cells, white blood cells, and platelets. Lupus can cause anemia (low red blood cells), leukopenia (low white blood cells), or thrombocytopenia (low platelets). Kidney and liver function tests check whether these organs have been affected by lupus inflammation. A test for antiphospholipid antibodies is sometimes performed because some lupus patients have these antibodies, which increase blood clot risk.

The presence of red blood cell casts or white blood cell casts is particularly suggestive of kidney inflammation from lupus. Normal urine contains little to no protein or blood.

The 24-hour urine protein test measures how much protein is lost in the urine over a full day. Healthy kidneys filter out waste while keeping proteins in the bloodstream, so protein in the urine indicates the kidneys are not functioning optimally. A result of more than 0.5 grams of protein per day may indicate lupus nephritis. More severe kidney involvement can produce 3 or more grams of protein daily.
