Your ear canal is self-cleaning. As you move your jaw—talking, chewing, and yawning—the skin in your ear canal gradually moves outward, naturally pushing old earwax toward the ear opening where it dries and falls out. This process works well for many people throughout their entire lives.

However, some individuals produce more earwax than others, or their ear canals are shaped in ways that make earwax accumulation more likely. According to research in the American Family Physician journal, approximately 1 in 20 people in developed countries experience impacted earwax at some point. In developing countries, the prevalence is higher, reaching up to 1 in 3 people in some regions, often due to environmental dust exposure and less access to removal tools.

Signs that earwax buildup may be affecting you include hearing that seems muffled or decreased, a sensation of fullness in the ear, ringing in the ears, ear discomfort, or itching inside the ear canal. Some people also experience dizziness or a persistent cough triggered by earwax pressing against the vagus nerve.

Ceruminolytic drops: Over-the-counter drops specifically formulated to break down earwax are available without prescription. Brands like Debrox and Murine contain carbamide peroxide, which is gentler than straight hydrogen peroxide and specifically designed for ear use. Follow package directions, which typically involve placing drops in your ear 1-2 times daily for 3-5 days. These drops work well for many people and can be found at any pharmacy.

Manual removal with proper technique: Once earwax has softened using one of the above methods, you may notice it beginning to come out naturally. If it doesn't, you can gently try to remove it. Never insert anything smaller than your pinky finger into your ear canal. Use a soft rubber ear bulb syringe (available at pharmacies) with warm water to very gently irrigate your ear canal, allowing the water to flow in and immediately drain out, carrying softened wax with it. These tools easily puncture the eardrum or scrape the ear canal skin, leading to infection and hearing damage. The ear canal is only about 1 inch long, and the eardrum sits at the end. It's far too easy to reach it accidentally with a pointed object, especially if you move suddenly or someone startles you while the object is inserted.

Candles: Ear candles are hollow tubes marketed as a "natural" earwax removal method. The claim is that lighting one end creates a vacuum that draws wax out. This method doesn't work—scientific research has found no evidence that ear candles create suction or remove earwax. Beyond ineffectiveness, they pose real burn risks. Hot wax can drip from the candle into your ear canal, and the flame can burn your ear or hair. Several cases of serious burns and eardrum perforation have been documented.

Compressed air or high-pressure water: Some people attempt to blow out earwax using compressed air tools or high-pressure water devices. This approach can rupture the eardrum, especially if the pressure is forceful. Even "gentle" pressure can be too much when the eardrum is already compromised by impacted wax.

Excessive or continuous irrigation: While gentle irrigation with saline or warm water is safe, repetitive or forceful irrigation can cause middle ear infection by forcing liquid through the eardrum or introducing bacteria into the ear canal. Never irrigate if you suspect your eardrum might be perforated.
